Attention MyChart Users

To better serve you, we are in the process of combining our West Bend computerized medical records system into the Froedtert computerized medical records system. Once completed, you will be able to log into one MyChart® account to view portions of your medical record across the health system; no matter where you receive your care. On September 1, 2013, your access on the West Bend instance of MyChart will be changed to “read-only”. You will need to create a new account in the Froedtert & The Medical College of Wisconsin MyChart® instance in order to see MyChart® information going forward.

Note, limited read-only access to the West Bend MyChart instance will be available until September 2014.

Enrollment Instructions

To enroll in the Froedtert & The Medical College of Wisconsin MyChart® instance: visit, click on “Request Activation Code”, complete the form and click “Submit.” You will receive an activation code for the new system in the mail within 7 to 14 days.

After September 7, you may also enroll at your clinic by asking to enroll in MyChart® at the Clinic Reception area. Our friendly patient representatives will be happy to assist with this fast, easy process. Once you receive your activation code, simply log into your computer and create your own MyChart® ID and password. Note, in some cases you may not be able to use the same MyChart ID you used on the West Bend instance.

Upon completion, you will have the same great access to the MyChart® tools you currently have in the West Bend instance.

West Bend Clinic MyChart Patients

Over the course of the next year, your historical medical record information will be transitioned onto the new MyChart version. If your information is not yet available on the new MyChart version, please refer to the previous MyChart version to view historical information. If you identify that your information is incomplete or inaccurate on the new MyChart and need to schedule an appointment, obtain a medication refill or contact your provider, please contact your physician’s office.

Note: The West Bend system will contain your historical information prior to September 7, 2013 and the new MyChart version will contain your information going forward. Your information in this system will be updated at your next primary care provider visit and/or as your information is transitioned over.
